17:2 Suppose a man or woman is discovered among you – in one of your villages ¹  that the Lord your God is giving you – who sins before the Lord your God ²  and breaks his covenant

19:5 Suppose he goes with someone else ³  to the forest to cut wood and when he raises the ax ⁴  to cut the tree, the ax head flies loose ⁵  from the handle and strikes ⁶  his fellow worker ⁷  so hard that he dies. The person responsible ⁸  may then flee to one of these cities to save himself. ⁹ 

Deuteronomy 21:17

The practice is implicit in Isaac’s blessing of Jacob (Gen 25:31-34) and Jacob’s blessing of Ephraim (Gen 48:8-22).
